- The Everyday Smiles Membership Plan is not dental insurance and cannot be combined with any dental insurance plans or third-party payers.
- Membership fees must be paid in full at the time of enrollment and are non-refundable.
- Plan benefits are valid only at Everyday Smiles Pediatric Dentistry and cannot be used at other dental offices or specialists.
- The plan is valid for 12 months from the date of enrollment (365 days).
- Discounts under the membership plan do not apply to products or services provided by third-party specialists or external labs.
- Membership covers routine preventive care as outlined in the plan details. Additional treatments will be billed at discounted member rates.
- Membership must be renewed annually to continue benefits.
- Plan benefits cannot be transferred to another patient or family member.
- Missed or canceled appointments without 24-hour notice may result in forfeiture of a benefit.
